How we work

Every assessment begins with a free introductory phone call to gain an understanding of your current situation. We then arrange an at-home or online assessment* so we can determine additional services and support that will be beneficial now, and into your future.

Examples of these services may include (but not limited to):

  • Referral to My Aged Care
  • Referral to private home care services
  • Recommendations to access health services such as GPs and physiotherapists
  • Discussion and recommendations for financial and legal decisions

*Home assessment may be converted to online assessment pending lockdown and isolation arrangements.

About Jane

After receiving her Bachelor of Social Work at the University of Sydney, Jane started her career as a Hospital Clinician for the Aged Care Assessment Team (ACAT) then moved on to a community provider where she worked as a Home Care Package Manager, NSW Business Development Manager and Assessor for the new My Aged Care Regional Assessment Service (RAS). 

Working closely with General Practitioners and Hospital Avoidance teams in her role at the Primary Health Network, she was heavily involved in building strategies to reduce risk of hospitalisations for older Australians.

More recently, she worked for a not-for-profit provider, building relationships across Sydney to increase client numbers and further develop capability to apply for Government Grants and Tenders.

Jane continues to work with providers supporting client growth and retention, grant applications, and staffing challenges.
